This applies to all users — including patients, members, athletes, and minors — and supplements our HIPAA policy for medical patients.

This includes our use of cookies, chat tools, SMS messaging, embedded third-party platforms, and analytics tracking. These terms apply to all users — including patients, members, athletes, and minors — and are intended to complement our separate HIPAA policy for medical patients and privacy policies for fitness and membership services.
Fitness & Athlete Participation Liability Waiver
This waiver is required for all participants engaging in athletic training, group fitness, performance sessions, gym access, or independent equipment use. It outlines the risks, expectations, and your legal release of liability. If you or your child are participating in any fitness or sports program at THE ARMORY, this waiver must be completed before your first session. This waiver is signed by clicking on the link below.
Patient Intake Agreement (Medical & Recovery Services)
This agreement outlines your rights and responsibilities as a medical patient of THE ARMORY. It includes your consent to receive care, our HIPAA Notice of Privacy Practices, our cancellation policy, and our financial terms for direct-pay care. This document is signed electronically during your first medical visit via our Elation health system.
Membership Agreement
This unified agreement governs all monthly memberships at THE ARMORY, including SHIELD, Medical, Fitness, and Recovery plans. It explains your access privileges, contract terms, cancellation rules, nonpayment policy, and expectations for member conduct. This agreement is signed in WellnessLiving when you purchase or renew your membership and remains in force until canceled per policy.
