THE ARMORY uses Elation Health, a trusted electronic health record (EHR) system, to manage all of our medical care. Through Elation’s Patient Passport portal, you can:

• Request prescription refills

• Securely message your healthcare team

• View allergies, drug Intolerance, medications & vaccines

• View visit summaries

• View date of upcoming appointment

• View date of last appointment

• Update your demographic information, allergies, drug intolerances,

     This is our preferred method for medical communication. Patient Passport is HIPAA-compliant and ensures your information stays protected. For prescription refills and medical questions, it is especially important that you use Patient Passport rather than regular email or phone calls, since we are required to handle requests through a secure, compliant system.

How to Access Patient Passport:
You can use Patient Passport in two ways:

• Web browser — click the login button below to open the secure website.

• iPhone or iPad — download the Patient Passport app from the App Store and log in with your account.
